CZUR is churning out some good book scanners and their proprietary software can automatically flatten out pages giving great high quality scans fast. i haven't tried it yet but as of writing this there are some campaigns on indiegogo that look excellent. 5. Once the scans are made they should be uploaded to archive.org AND library genesis. archive.org is good because you can go through the uploaders profile and browse and find things that you didn't know existed - but the down side is that they DO delete books that infringe on copyright. Library genesis is bad in that it doesnt allow browsing of the uploader's collection but the main positive is that they don't take anything down - so if you are going to upload anywhere then here on library genesis should be the priority.
